The Autzen Foundation Grant Application:
Thank you for your interest in the Autzen Foundation. The deadlines are March 15, August 15 and November 15. Application must be received by the 15th, even if the date is a Sunday. You may download the application here. The application is a MicroSoft Word doc designed to be printed, completed (no more than 3-4 pages in length), and sent by mail. When sending your application and other materials (including audits, 990, etc.) please DO NOT staple, or use binder or paper clips. In addition please do not fold your application. Application requests for $25,000 or more requires the completion of an additional financial worksheet (MicroSoft Excel file). Please print final document on 8X14 paper. If request is less than $25,000, you do NOT need to fill out the excel spreadsheet. We always appreciate receiving applications early.
